Flourish Elevate
One in-person visit per year required
- Comprehensive primary care (acute, chronic, preventive)
- Unlimited visits with no caps
- Direct, ongoing provider access between visits — portal, text, or phone call

Flourish Foundation
100% telehealth available
- Up to 12 follow-up visits per year (telehealth or in-person)
- Direct, ongoing provider access between visits — portal, text, or phone call
- GLP-1, HRT, TRT prescribing when clinically appropriate
- Nutrition and behavioral coaching
- Lab ordering and review
- Body composition analysis with Seca Bioelectrical Impedance Analysis (BIA) (in-person only)
- Requires a separate primary care provider
Tier-specific questions
Flourish Elevate
Is Elevate a replacement for my primary care?
Yes. Elevate is designed to serve as your primary medical home. That said, recognizing when additional expertise is an important part of high-quality care. When concerns fall outside her scope or warrant specialist involvement, Mackenzie will facilitate appropriate referral and coordination of care.
How often will I see Mackenzie?
As often as needed. There are no visit caps, and same/next-day appointments are available when clinically needed.
Is the initial consultation included?
No — the one-time $300 Initial Consultation is required for both memberships, separate from the monthly fee. It also covers your first month of membership.
Can I receive care if I travel or live outside Indiana?
Possibly. Telehealth follow-up visits may be available outside Indiana, subject to state regulations. Because Elevate serves as your primary care home, an annual in-person physical exam is required in Indiana.
Do you prescribe opioids?
No — opioid medications are not prescribed at Flourish. Our practice is dedicated to metabolic health, hormone optimization, and root-cause care.
Flourish Foundation
Is the initial consultation included?
No — the one-time $300 Initial Consultation is required for both memberships, separate from the monthly fee. It also covers your first month of membership.
Can I do everything from home?
Yes — Foundation is fully telehealth in Indiana. Outside of Indiana, care is available subject to Mackenzie's licensure and state telemedicine regulations.
Why do I need a separate PCP?
Foundation focuses on metabolic and hormonal care. Routine primary care lives with your existing PCP. You may upgrade to Elevate to make Mackenzie your primary care home at any time.
What if I want to upgrade later?
You can move from Foundation to Elevate at any time with a brief transition visit.
Can I join from outside Indiana?
Possibly. Foundation's telehealth visits may be able to accommodate patients residing or traveling outside Indiana, subject to state telemedicine regulations.
All new patients begin with a one-time $300 comprehensive Initial Consultation, which also covers your first month of membership. Labs and medications billed separately.
