How does care begin?

Every patient begins with a 60-minute Initial Consultation ($300). Labs are ordered ahead of time so we review results together during the visit. You choose whether to process labs through insurance or pay cash.

Is Flourish actual medical care, or a wellness program / med spa?

Flourish is a licensed medical practice led by a doctorally prepared, board-certified Family Nurse Practitioner. We diagnose, treat, order labs, and prescribe medications with full clinical accountability.

Is this in-person or telemedicine?

Both. Visits are available via telemedicine or in-person in Indiana. Outside of Indiana, telemedicine is available subject to Mackenzie's licensure and state telemedicine regulations. Elevate requires an in-person Initial Consultation and an annual in-person comprehensive physical exam. If you upgrade from Foundation to Elevate after completing your Initial Consultation via telemedicine, we will schedule your in-person physical exam.

Why is the initial consultation charged up front?

The fee protects provider time and covers labs ordered at booking. Rescheduling is always free.

What's the difference between Elevate and Foundation?

Elevate is full primary care plus metabolic and hormonal care. Foundation is metabolic and hormonal care only, and assumes you have a separate primary care provider. You can switch at any time.

Can I change my membership type at any time?

Yes — you can switch between Elevate and Foundation at any time. If you prepaid, the switch applies at your next renewal; prepaid memberships are nonrefundable.

Is there a discount for paying up front?

Yes — 5% off membership fees for 6 months paid up front, and 10% off for 12 months. The discount applies to membership only, not the Initial Consultation. (The consultation does include your first month of membership.) 6-month Elevate: $1,425. 12-month Elevate: $2,700. 6-month Foundation: $855. 12-month Foundation: $1,620. Prepaid memberships are nonrefundable.

Can I cancel my membership?

Month-to-month memberships are able to be cancelled with 30 days' written notice. Prepaid memberships are nonrefundable.

Do you take insurance?

Flourish is cash-based for visits and membership. We encourage using insurance for labs, diagnostics, and medications to keep your out-of-pocket costs down.

Do you accept Medicare, and what about medications?

We do not bill insurance, including Medicare. Care is provided through a monthly membership. If you have Medicare, you're still welcome to join. You'll sign a private agreement confirming that neither you nor our practice will bill Medicare for services provided. Prescriptions written through our practice can still be filled using your Medicare Part D coverage at the pharmacy.

Can I use HSA or FSA?

Yes — the Initial Consultation, labs, and medications are typically HSA/FSA eligible. Monthly membership fees generally are not. Check with your plan administrator for specifics.

Can men join Flourish?

Yes. We offer testosterone replacement therapy (TRT) and full metabolic care for men.

I'm already on medication from another provider. Can you take over?

Yes. We review your current regimen, order baseline labs, and transition your care when appropriate.

Do you prescribe GLP-1, HRT, and TRT?

Yes — GLP-1 therapies, hormone replacement therapy (HRT), and testosterone replacement therapy (TRT) are prescribed when clinically appropriate.

Do you offer peptides?

We provide FDA-approved peptide therapies, including GLP-1 medications, when clinically appropriate.

Do you offer testosterone pellets?

No — we do not offer T pellets for men or women.

What is the cancellation / reschedule policy?

We appreciate 24 hours' notice for reschedules or cancellations, but there is no fee.

What if I haven't been seen in over a year?

A new Initial Consultation fee will apply so we can reassess your current needs before restarting care.
